Output Instrument Samples

Outputs track the completion of activities, and document the fact that products were created and/or individuals received program services. Outputs answer the question “How much service did we do?” Outputs do not measure changes as a result of that service. Output instruments are often simple logs and attendance sheets.

Below are links to some output instrument templates. Download any of these Word documents and modify them to fit your purpose.

Output Instrument Samples for Tracking of Ongoing Service/Contact:

Training Attendance Log can be used to track the number of trainings, sessions, or events attended by a group of expected participants (e.g. members, community volunteers, or students). (Click here for an Excel version of the Training Attendance Log).

Client Services Log can be used to track the names and the number of people receiving ongoing services, and the amount of service each receives (e.g. students attending homework help sessions; seniors receiving weekly groceries).

Meal Delivery Log can be used by volunteers to track deliveries to individual homes, indicate any problems encountered, and record the number of meals each client receives.

Client Transportation Record can be used by volunteer drivers to track clients served, locations and times of service, miles accumulated, and any problems encountered.

Ongoing Meeting Log can be used to track the number of regular meetings between two individuals (e.g. mentor and mentee, tutor and student, visitor and homebound senior) and some details of what occurred at the meeting, such as meeting duration and activities.

Community Meeting Record can be used to track the dates and frequency of group meetings held (e.g., Neighborhood Watch) and the number of attendees.

Output Instrument Samples for Tracking of One-Time Service/Contact:

Attendee Sign-in Sheet can be used to track the number of attendees at a presentation or training, or participants of a drop-in activity.

Community Volunteer Sign-up Sheet can be used at a recruitment activity to track the number of community volunteers recruited and collect contact information.

Client Assistance Record can track the number of one-time meetings between a volunteer/member and individual clients assisted (e.g. tax counseling assistance, emergency referral or disaster relief assistance), with some details of what occurred such as the type of assistance delivered and whether follow-up is needed.

Presentation Log can be used to track the number of presentations given and the number of one-time attendees at each, and collect additional details if needed (e.g. type of materials distributed, number of community volunteers recruited).

Household Contact Log can be used to track the number of households contacted, locations, and type of service delivered (e.g. households receiving recycling bins).
